DUTIES: Develops and monitors Individual Treatment Plans, and ensures that clients are receiving and integrated program of services that provides a high probability of treatment success. When necessary, the Case Manager makes and monitors referrals for other services. Responsible for monitoring the delivery and coordination of a broad range of service to clients. Works with court personnel and outside agencies; develops individual treatment plans with each client; coordinates all intake services; follows up on referrals; builds collaborative agreements; ensures that all provided services are recorded; meets regularly with Therapist to monitor clients’ progress; provides guidance on treatment issues to Counselors; and establishes procedures for ensuring a high level client confidentiality.
MINIMUM QUALIFICATIONS: A minimum of Bachelors in Psychology, Sociology, I/O Industrial Psychology, Counseling, or other social services major. Three years experience in social services or mental health, knowledge and understanding of the Latino community and drug and alcohol abuse problems and treatment, mental health, and bilingual/bicultural.
