PRIMARY DUTIES:
- Performs case analysis studies, assigns case staffing, and monitors and evaluates case histories.
- Reviews case files for completeness of documentation, as well as, compliance with established policies, guidelines, and professional practices.
- Convenes case staffing meetings and leads case review sessions.
- Establishes and maintains a program for case management and record keeping.
- Obtains and records relevant and significant facts from case workers, program participants, service providers, etc., and obtains additional information when required.
- Determines and/or assesses the feasibility of alternative plans of intervention and/ or social service delivery systems.
- Acts as the District’s representative when monitoring and coordinating the work of contracted social service delivery agencies. Assures that agencies are delivering quality services to families and children and that these services are in compliance with the Department of Child Protective Services regulations. Reviews agencies case notes to determine progress and service delivery; conducts meetings with agency representatives or other related groups; and conducts meetings with parent groups.
- May make on site or collateral visits to obtain case information.
- Performs related duties as assigned within classification.
